Medical Plan
Peak offers a very generous PPO (preferred provider) plan provided by Cigna.  By using a network provider your benefits are paid at a higher level. No referrals are needed to seek specialty care, but preauthorization may be required for some treatments and procedures.

This plan is a high deductible plan with an employer sponsored Heath Reimbursement Account (HRA).

The Drug Prescription benefit is part of the medical plan. We offer both retail and mail-in programs.


Dental Plan
You may select any network dentist in the state where you work or reside through Delta Dental.


Vision Plan
You may select any network provider in the state where you work or reside.


Basic Life/AD&D
Peak provides basic life and accidental death and dismemberment in the amount of $50,000 at no cost to you, through Mutual of Omaha.  We also offer voluntary guaranteed life if employee enrolls within the first 30 days of employment. 


Long Term Disability
Peak provides LTD at no cost to you, through Lincoln Financial Group. Maximum monthly benefit payable is $8,333.


Flexible Spending Account
Pre-tax accounts that allow participants to pay for a wide range of medical, dental, vision expenses not fully paid for under insurance plans. You are not required to have medical coverage with Peak to participate in the FSA.
The dependent care account allows participants to pay for eligible dependent care expenses with pre-tax dollars.
 

Retirement
Peak offers a voluntary pre-tax retirement account as well as Roth (after tax) and college savings accounts.

Peak contributes 3% after the first year of employment, 4% after two years and 5% after the third year. Employee need not participate to receive employer contribution.
