Get Started To begin services at Peak, we will start with an intake appointment, which is an "information gathering" type of appointment so that we can get to know you, and understand what brings you in to see us.
The first people you see at Peak will ask to learn more about your situation through a clinical assessment, and by the end of the appointment they will match you with a therapist who is qualified to help you with your specific situation. (So your second visit is when you will actually begin treatment with your therapist.)
At this first visit, you will need to complete your client application and participate in a clinical assessment. We will also set your fees in accordance with our sliding fee schedule. Once your assessment is complete we will have the information we need to make the best treatment recommendations and you will be placed with a therapist for group or individual services.
What is a clinical assessment?
During the clinical assessment, one of our qualified therapists will ask you questions about your growing up experiences, your current situation, what kind of problems you have had or may be having currently, and other questions to help us understand your situation, so that together we can make the best possible decisions about your care.
To expedite your first visit (intake), please download and complete your application and health history and bring them with you, along with the following:
- Proof of income for all household family members (if applicable)
- Proof of insurance (if applicable)
- Court documents, if applicable (such as court orders, probation or parole documents)
We cannot complete your application without this information. Without the required information, your appointment may have to be rescheduled, or your fee may be set at the standard rate $120 fee.
Fees
Since Peak Wellness Center charges on a sliding fee scale, your fee is based on income and your ability to pay. Please report any changes in your income or financial status, including the number of people dependent on your household income, as soon as possible.
Sharing your information
In the event that you would like Peak Wellness Center to release your information or receive information from outside sources, such as attorneys, family members, etc., please bring the exact name of the person or agency, current address, phone and fax numbers to your appointment and we will ask you to complete a release form.
